Donation Forms

SNAP provides a lifeline to families in the Midlands and Staffordshire who have children with special needs and disabilities but is entirely dependent upon donations for its continuing existence. All donations are very gratefully received and make a huge difference to the Charity.

If you would like to make a donation, however large or small, please print off and complete the Donations Form below and return it to SNAP. If your donation is for a particular appeal, please write the name of the appeal on the form.

If you would like to make a standing order to SNAP, please print off and compete the Banker's Order Form below and return it to SNAP.

If you pay tax, SNAP can reclaim the tax you have already paid on your donation. This is currently 28p for each £1 donated. So if you would like to maximize your donation to SNAP without it costing you a penny more, then please print off and complete the Gift Aid Form below and return it to SNAP with your donation or Banker's Order Form.
